Clinical Analysis of Patent Ductus Arteriosus

Abstract
|
|
|
43 cases of isolated patent ductus arteriosus were operated from June, 1982 to May, 1985.
Clinical analysis of these patients were as follows;
1. Sex ratio, male: female, was 13:30.
2. Mean age at operation was 11.31¡¾6.08 years. The youngest patient was 2 years and the oldest one was 25 years.
3. The most common symptom was frequent URI(53.1%).
4. Continuous machinery murmur could be heard at the 2nd or 3rd intercostal space on the left sternal border in 36 patients (83.7%). The other 7 patients made systolic murmur with accentuation of 2nd heart sound and those were associated with pulmonary hypertension.
5. Radiologic findings of chest were cardiac enlargement in 19 patients(44.2%), enlargement of pulmonary conus and increasing density of pulmonary vascularity in 25 patients (58.1 % )
6. Electrocardiographic findings were within normal limit in 13 patients(30.2% ) .
7. Echocardiogram was done in all patients. Ductus was directly visualized in 8 patients of 18 patients in doing sector scan.
8. Cardiac catheterization was performed in all patients. The mean value of the results were: Systolic pulmonary artery pressure=40.11¡¾18.62mm Hg, Diastolic pulmonary artery pressure= 20.32¡¾14.97mmHg, S02(PA-R V)=12.76¡¾3.93%, QP/Qs=2.25¡¾0.44.
9. All patients were operated through the left posterolateral thoracotomy and the most common post-op complication was hoarseness.
10. Mortality rate was 6.9% (3patients), and the cause of death was mainly aneurysm rupture of operative site.
